Full Job Description
To work alongside the Registered Care manager to manage the Home day to day.
- To take full professional accountability in respect of the clinical management of the home.
- To making initial evaluation visits as applicable and to re-evaluate each Resident's nursing needs on a regular basis.
- To providing nursing services, treatments and diagnostic procedures, requiring specialised skills and assuming full responsibility for each of Resident.
- To develop and implement the treatment plan for each Resident under the direction of their physician.
- To observe and report symptoms, reaction to treatments, drugs, and changes in the Resident's physical or emotional condition.
- To initiate preventative and rehabilitative nursing procedures as appropriate for the Resident's care and safety.
- To maintain clinical and progress notes for each Resident receiving care and providing progress reports to their physician.
- To coordinate services for Residents as needed with other agencies and health care providers as appropriate.
- To carry our Risk Assessments for Residents and to update them as and when required. To convey all updates to the Registered Care Manager.
- To conduct Medication Audits monthly or as and when required.
- To work in accordance with a rota that supports meeting the care needs of the home. This will include working alternate weekends, evenings, bank holidays and/or any other shift pattern that has been established by the Registered Care Manager based on the needs of the care home.
- To report observed or suspected adult abuse pursuant to mandated requirements.
- To assist the Registered Care manager in decision making and policy formation as it pertains to the Home Care Program.
- To adhere to all home care polices, including but not limited to; infection control and safety, education, reporting and practice.
- To demonstrate leadership of your staff team and to act as a positive role model ensuring your presence is visible within the community of the home.
- To ensure that each resident receives care appropriate to their individual need.
- To ensure all Care Plans are outcome based and have the needs of the Residents at the forefront of any planning.
- To liaise with GP practices, Dieticians, SALT team etc as and when needed and to ensure all paperwork is logged accurately.
- To ensure that effective assessment of staff is carried out and that training needs are identified and met within Budget.
- To make or contribute to the assessment of needs of each Resident in conjunction with the Resident, relevant professional agencies and, where appropriate, the Resident's family, and develop a care plan which provides a satisfactory quality of life for that person.
- To ensure that all care plans are reviewed regularly and kept up to date in accordance with CQC and company requirements.
- To promote relationships which enable each Resident to participate in social activities within the home and in the life of the local community to the maximum of their ability.
- To undertake regular clinical audits such as pressure ulcer, accidents and care.
- To ensure that effective induction, supervision and assessment of staff is carried out and that training needs are identified and met.
- To ensure that each member of staff is aware of the Home's policies and has recorded their awareness in the company documentation.
- To ensure that each member of staff is providing safe and good care for each resident by following them Company Policy and Procedure in respect of staff supervision.
To assist in the overall implementation, monitoring and reviews of practices and systems in the home.
To effect changes in environment to ensure Resident safety and security.
To support Residents in the taking of decisions in matters which affect their lifestyle.
