Full Job Description

As a Reablement Worker you will work closely with our Therapy and Support Leads, Registered Manager, and other professionals to ensure a high level of person-centred care is delivered using a therapy led, individualised reablement care and support plan and risk assessments.

You will be responsible for facilitating admissions to the service, which will include liaising with families and other agencies to ensure a smooth transition to Borrowdale House.

Administration of medication according to policies and procedures, low level health care tasks and implementation of therapy guidance will be required, ensuring best practices are always applied and in the best interests of the person you are supporting.

Have you previously worked as a Reablement Worker or Support Worker, with experience of supporting people to regain their independence following a stay in hospital?, We are looking for an experienced Reablement or Support Worker, who are passionate about person centred support with a dynamic innovative approach to join our new team at Borrowdale House., You will have experience of working in a health and social care environment, as a reablement or support worker. With excellent coordination and communication skills, be able to deliver personalised outcome based reablement plans and be able to develop and maintain effective relationships with a wide range of stakeholders.

You will need the confidence and ability to demonstrate sound professional judgements, when making decisions and have the ability to support real change.
