Advanced Nurse Practitioner / Advanced Care Practitioner

Undertake Clinical Nursing Practice at an advanced level to deliver holistic care to people accessing primary health care services. Assess, diagnose and treat patients contacting primary health care services with a range of acute and chronic medical conditions. Prescribe medicines and appliances as determined by patient need. Directly admit patients to secondary care hospital in acute medical need. Refer patients to other medical specialities or to other members of the multidisciplinary team for assessment if deemed appropriate. Interpret diagnostic tests and routine clinical procedures. Advise and support others as needed. Work within professional guidelines and codes of conduct, acknowledging own limitations. Work within all relevant practice policies and procedural guidelines, e.g. infection control. Chaperoning, risk management. Independent Nurse prescriber. Contribute to the practice targets e.g. QOF, ReVivo. Contribute to disease registers. Contribute to vaccination campaigns. Please
